What’s Included

A thorough, individualized look at what is changing

Each evaluation is built around the patient in front of us. Dr. Van Horn goes over what to expect at the time of scheduling. Typical components include:

  • 01 · Thorough history review

Thorough review of medical history, current medications, and patterns of daily functioning.

  • 02 · Patient and family interviews

Unhurried clinical interviews with the patient, and with family members, with permission, when their perspective helps clarify what has been changing.

  • 03 · In-person clinical observations

Clinical observations of the patient in person, so what we hear and what we see are taken together.

  • 04 · An individualized written plan

An individualized written plan that explains what we found, what it may mean, and what next steps may be appropriate.

What This Evaluation Is For

Memory, thinking, behavior, taken seriously, taken together.

Cognitive and memory symptoms rarely fit a single, simple explanation. Medication effects, mood, sleep, neurological factors, and life circumstances can all play a role. The Comprehensive Cognitive & Memory Evaluation is designed to consider the whole clinical picture, not a single symptom in isolation.

You leave with a clear, written plan explaining what we found, what it may mean, and what next steps may be appropriate for your situation.

When This Evaluation Is Helpful

  • When several possible explanations exist

Symptoms that could reflect medication effects, mood, sleep, neurological factors, or something else entirely.

  • When prior evaluations have not produced answers
